Only the sides finishing in the top two of Group A1 will qualify for the finals, with the fourth-placed nation suffering the agony of relegation.
As it stands, Portugal are top on 10 points and due to claiming a 3-1 win when the sides met last month, they will seal a place in the next phase as long as they avoid defeat against Poland by more than one goal.
Alternatively, qualification for Roberto Martinez’s side will be secured if Croatia lose to Scotland.
For Poland – who will be without all-time leading scorer Robert Lewandowski through injury – a win over Portugal coupled with Scotland failing to beat Croatia will not only see them retain hope of securing a finals spot but also ensure they avoid relegation.
However, defeat at the Estadio do Dragao in Porto would end any hope of qualification and relegation will still be on the cards if the result of the other game goes against them.
